The AMA provides multiple options for submitting this release, including an online version. Please select the option that is right for you. Please Note: Annual Minor Release Cards are only active for the calendar year and will expire on December 31st of every year. If you submit a minor release waiver at the end of 2019, you will be required to submit a new one in January of 2020 for the minor to continue racing.
If you are submitting an annual minor release prior to competing in an event, that release must arrive at AMA offices at least 3 weeks before the event to allow time for processing, printing, and shipping. Printed forms should be notarized and mailed to the following address:

1) Electronic Annual Minor Release Process (Preferred Method)
The Electronic Annual Minor Release form can now be completed online utilizing the links listed below. Upon submission, you will receive an email confirmation and will be required to reply.
Both Parents/Guardians Version – The process requires each parent/guardian to sign and submit a form individually to complete the process.
Sole Custody Version – The process requires the parent/guardian with sole custody to sign and submit only one form to complete the process.
2) Printable Annual Minor Release Process
The Printable Annual Minor Release form may be submitted by completing the fillable document utilizing the link below, having the signature(s) notarized, and mailing the original form to the address provided above. It is important to provide all the requested information. In addition to the parent/guardian name and signature, there is a requirement to initial the sole custody parent/guardian box or provide the second parent/guardian name and signature.
3) Organizer Annual Minor Release Process
The Organizer Annual Minor Release form is available at the link below for use at an event. It is important to provide all the requested information. In addition to the parent/guardian name and signature, there is a requirement to initial the sole parent/guardian box or provide the second parent/guardian name and signature. The forms must be witnessed by an Event Official or a Notary.
